For adenocarcinoma of the prostate local control of tumor is directly
related to radiation dose. The challenge has been for physicians to increase
the tumor dose while at the same time minimizing the volume of normal tissue
within the high-dose volume. One avenue is the use of three-dimensional
conformal radiotherapy, utilizing CT imaging to shape and design radiation
fields that conform more closely to the shape of the prostate. By use of
more tailored radiation beams the radiation dose can be increased without
increasing the complication rate(s). Another avenue of dose escalation is
a brachytherapy boost. To date, for patients with more advanced disease
(Gleason score greater than 7, or PSA greater than 10 ng/ml, or Stage T3
disease) external beam radiation therapy in combination with a brachytherapy
boost has offered some of the highest cure rates to date. This is an area
of active investigation at Thomas Jefferson University.
